- Essay:Please submit an essay on any subject of your choice, but keep your essay to no more than 500 words. Type essay on separate sheetof paper and include your full name in the upper right corner.
- Recommendation Letters:Include at least two letters of reference (by a teacher, principal, employer, etc.)
- Other Materials:1 List all community volunteer activities and years involved (i.e. church, local mission, etc.).2. List other activities such as Boy Scouts, Girl Scouts, bowling offices held, etc.3. List any scholarships you have received by name and amount. If they are managed through SMART just state SMART and totalamount in SMART account.4. List any activities not covered in the above questions pertaining to community involvement1. List the name of your current high school, your current grade and your expected graduation month and year.2. List your current grade point average and on what scale it is based.3. List any AP/Honors courses you have completed or are currently taking.4. List all scholastic honors or awards (i.e. National Honor Society, Honor Roll, 1st-place in science competition, etc.).5. List all extracurricular activities (i.e. school clubs, newspaper, yearbook, etc.).6. List any high school and college sports played and years played.7.
