    Plan to pursue an education in the medical field at any level.

    Selection will be based, in order of importance, on financial need, citizenship, and achievement

  • Recommendation Letters:
    Two letters of recommendation
  • Other Materials:
    Proof of GED, if applicable;Paragraph outlining your reasons for applying and an explanation of your need;Completed College Costs Estimator form;All applicants must submit one high-quality, color head shot (shoulders and up). A professional senior photo is preferred. HCCF reserves the right to request additional photos if the submitted photo is not acceptable. *Please note, if you are selected as a scholarship recipient this photo will be used in HCCF publications
