- Age/Grade Level:Available to high school seniors graduating in the spring of 2016 and students who will be an undergraduate or graduate student during the 2016/2017 school year.

- Essay:A short personal statement describing the applicant's academic aspiration and other interests. This should describe why the applicant chose the proposed area of study (major), the applicant's aspirations post-graduation, definition of success and the reasons why they wish to be a recipient of the Cochlear Graeme Clark Scholarship. This statement must not exceed 1,000 words and should be written in as simple and direct a manner as possible. The applicant must sign the statement in ink.One photocopy of the applicant's birth certificate, passport or other proof of age and citizenship.
- Recommendation Letters:The letters of three references. (At least two of these must be someone from whom the applicant has received educational instruction.)
- Other Materials:The applicant's university admissions notice (attach copy).A succinct list of the principal activities and awards in school (including prizes, scholarships, offices held, athletic record, extracurricular interests and substantial activities) with dates. These must not exceed one page in length, and should be set in a typeface no smaller than 10 points.Written proof of residence (photocopy of driver's license).
